Regulatory Compliance
Guardian Community Care operates as a licensed non-medical home care agency in Ohio and follows all applicable regulations.
We comply with requirements established by the Ohio Department of Health (ODH) and the Ohio Department of Aging (ODA). Our compliance program covers onboarding, service delivery, record keeping, and reporting to make sure every visit meets state expectations and client needs.

- Immediate reporting of critical incidents in line with state guidelines
- Maintenance of secure records for the retention period mandated by Ohio law
Licensing & Oversight
Our licensure status is kept current and reviewed regularly to ensure we meet statewide standards for non-medical agencies.
Licensing Commitments
- Active home care agency license issued by the state of Ohio
- Timely renewal filings with documentation of policies and procedures
- Cooperation with state surveys, inspections, and quality reviews
We maintain certificates of insurance, business registrations, and other credentials that may be requested by clients, partners, or regulators.
Service Scope & Limitations
Our caregivers deliver supportive, non-medical services designed to help clients live safely and comfortably at home.
Services We Provide
- Personal care such as bathing, grooming, and mobility support
- Companionship, social engagement, and respite for family caregivers
- Homemaking, laundry, light housekeeping, and meal preparation
- Medication reminders consistent with non-medical regulations
- Transportation and accompaniment for errands or appointments
Outside Our Scope
- Skilled nursing, medical treatments, or medication administration
- Diagnosis, clinical assessments, or medical decision-making
- Therapies that require licensed professionals (PT, OT, etc.)
- Tasks that pose safety risks such as heavy lifting or hazardous cleaning
Service Area: Lake, Cuyahoga, and Geauga counties in Ohio. Requests outside of these counties are considered on a case-by-case basis and may require additional lead time.
Insurance & Liability Coverage
We carry comprehensive insurance to protect clients, caregivers, and the organization while services are delivered.
- General liability insurance for incidents that may occur during scheduled visits
- Professional liability coverage for agency operations
- Workers’ compensation coverage for all employed caregivers
Clients should maintain their own homeowners, renters, or medical insurance coverage. Guardian Community Care is not responsible for conditions or incidents that fall outside of the agreed-upon services or scheduled visit times.
Employment & Training Standards
All caregivers are direct employees who receive supervision, training, and performance reviews.
Hiring & Onboarding
- Criminal background checks, reference verification, and driving record reviews
- Orientation covering agency policies, emergency procedures, and client rights
- Probationary supervision with check-ins from our care coordination team
Ongoing Training & Support
- Annual in-service training that satisfies Ohio non-medical home care requirements
- Specialized education for dementia care, fall prevention, and infection control
- 24/7 on-call support for urgent questions while caregivers are on shift
Service Agreements & Policies
Client services are governed by a written Service Agreement that outlines expectations for both parties.
- Customized care plans that describe tasks, visit frequency, and safety considerations
- Billing schedules, rates, and cancellation terms tailored to client needs
- Procedures for reporting changes in condition or adjusting the schedule
- Steps for resolving concerns or terminating services if expectations change

Privacy & Confidentiality
Protecting personal information is essential to our care delivery and compliance responsibilities.
- Client records are stored securely with role-based access for authorized staff only, including encrypted Caresmartz360 workspaces dedicated to PHI.
- Information is shared with third parties solely with client consent or when required by law, and only after confirming contractual safeguards such as Business Associate Agreements (BAAs).
- Caregivers sign confidentiality agreements, receive HIPAA-adjacent privacy training, and follow vendor oversight procedures that cover partners like Google Workspace (under our executed BAA).
